All FCC clinics have trained and certified community health workers who serve as patient navigators to assist you with the following applications:
- Medicaid
- CHIP
- SNAP
- TANF
- Long Term Care Services
- Healthy Texas Women (HTW)
- Health Insurance Marketplace
FCC staff will assist you with connecting to agencies that provide housing, clothing, and other assistance.
Please call 903-535-9041 for an appointment or simply walk into any FCC clinic and we will gladly assist you. Our community health workers are trained professionals who can offer guidance, support, and advocacy to individuals and families, helping you navigate complex systems. We provide services in all languages, including sign language.
FCC hosts an East Texas Food Bank food distribution the third Friday of every month from 10:00am to 11:30am at our West Gentry Clinic located at 2231 West Gentry Parkway in Tyler. About 600 families are provided with family food baskets each month. No identification or application is necessary. On each food distribution date, FCC also provides primary care from its mobile medical unit in the parking lot.
Qualifications that Enable Us to Assist You
Family Circle of Care is a proud participant in the Community Partner Program (CPP), a collaboration between Texas Health and Human Services and community agencies like FCC. The purpose of the program is to help people apply for and manage their medical, nutritional, and cash assistance benefits using the “YourTexasBenefits.com” online application website. HHSC benefits programs include Medicaid, Long-term Care Services, Children’s Health Insurance Program (CHIP), Healthy Texas Women (HTW), the Supplemental Nutrition Assistance Program (SNAP), and Temporary Assistance for Needy Families (TANF), and other potential benefit programs. One of the goals of the program is to provide places, outside of the HHSC eligibility offices, for people to apply for HHSC Program benefits online. FCC is a Level 3 Partner and functions as an application assistance and case management site to help you apply for, renew, and manage your benefits online. Our staff members who provide these services are certified by HHSC as Case Assistance Navigators.
Family Circle of Care employs staff members who are Certified Application Counselors for the Health Insurance Marketplace. We will help you find a plan that is best for you.
