FCC offers affordable care for people in all life stages, financial, and insurance positions.
We take most private insurances, Medicaid, and Medicare. We also have a Patient Discount/Sliding Fee Scale Program (English | Spanish) for uninsured and underinsured patients. We understand that a patient’s funding and insurance source may change many times throughout your life. Since we welcome all payment sources, you can continue to remain a patient at FCC as your life changes.
If you do not have insurance and need to see a provider, please call and make an appointment. When you come in, we will assist you with applying for a public health insurance program or other coverage you may qualify for. People who are not eligible can receive care using our Patient Discount/Sliding Fee Scale Program (English | Spanish).
Family Circle of Care offers a Patient Discount/Sliding Fee Scale Program (English | Spanish) for patients with low incomes without insurance or with insurance that does not adequately cover doctor visits and prescription costs.
For Office Visits – For patients under 200% of the poverty guidelines, an office visit will cost $15, $20, $25, or $30, depending on household income and the number of people in your household. Your individual fee is based on the Sliding Fee Scale.
For example, please look at the yellow highlighted area of the Sliding Fee Scale below. In the example, if there are 4 people living in your home and your total household income is $35,000, you fall on Slide B ($30,001 to $45,000 for 4 people in your home). Therefore, your payment is $20 for a medical provider visit, counseling visit, or dental visit. You can find your fee for different numbers of people in the household and household incomes by referring to the Sliding Fee Scale chart. When you come for your visit, our front desk staff will assist you with determining your fee.
Anyone can apply, including patients with insurance. If you apply and qualify, you will pay the lower of your insurance co-pay and the Patient Discount/Sliding Fee Scale Program amount. For instance, if you qualify for $15 and your private insurance co-pay is $45, you will pay the lower of the two so your fee will be only $15.
If you make more than the amount on the chart based on your household income, you are still eligible for a discount - If you pay the same day, you will get a 50% discount off the cost of the visit.
To apply for the Patient Discount/Sliding Fee Scale Program, you will be asked to complete a short (application) and provide documents that show the following:
Please note that assets such as bank statements and home ownership are not counted as income. Non-cash benefits such as SNAP and housing subsidies also do not count as income.
If you do not have the above documents available, you will still be provided with medical care. The amount you pay will be based on the income and number of people in your household you declare. You will be asked to bring the requested documentation to your next appointment. You do not have to be a citizen to be seen and there is no residency duration requirement.
If you are a Family Circle of Care patient and do not have Medicaid, you may qualify for a discount on certain medications prescribed by our clinicians.
The following local pharmacies offer a discount through a program called the 340b pharmacy discount program for medication prescribed by Family Circle of Care clinicians:
Please note that not all medications are included in the 340b discount program. Please ask the pharmacies above if your medication is included in the program and if not, if they have another discount rate that may apply to you.
For certain Family Planning visits, you may qualify for a Title X discount. An ID is not required for family planning visits but an ID might help with additional program applications.
In certain instances, women and children who are not covered under Medicaid may qualify for Title V fee-for-service program. It can help cover some medical and dental services for women, during and up to 90 days postpartum, and children and adolescents, birth to age 21. The front desk can provide you more eligibility information. Forms for Title V program are below:
For certain OB/GYN preventive services such as cervical cancer and mammogram screening, you may qualify for a Breast and Cervical Cancer Services discount (English Application | Spanish Application). The front desk at your clinic will help you sign up for programs for which you are eligible.
For Labs and Other Services Provided Onsite by Vendors
Please also note that Trincare, the labs providing lab work at our clinics also have discount programs. You may ask for information about their discounts at our front desk. For sonograms provided by Sonocare, there is also a reduced rate for eligible patients.
